The current order form reflects an increase in the shipping & handling fees due to 2006. postal rate increase and increases in shipping supplies. It also reflects the new state sales tax rate of 6.75 per cent effective Dec. 1, 2006.
The prices and fees on this form supersede any prices and fees printed on forms included with books printed prior to June 30, 2002.
I have made every attempt to make the retail order form fit on one page. However, there are too many variations in browsers and possible browser configurations to guarantee this.
Whether or not the form will fit on one page will depend in part on how you have the margins set for printing and the default font sizes selected for viewing Web pages.
With Microsoft Internet Explorer, margins are set by selecting File -> Page Setup.
With Netscape Navigator, margins are set by selecting File -> Page Setup.
Suggested margins for Page Setup are 0.5 inch top and bottom; 1.0 inch left and right.
In Microsoft Internet Explorer, you may want to set a smaller font than you normally use for viewing Web pages.
Select: View -> Fonts -> Small.
